Important Enrollment Information:
- Subscription Activation: Activation: Your subscription will commence only after your lab
work has been evaluated and you are medically eligible or the program. - Minimum Commitment: The subscription requires a minimum commitment based on the plan term selected (i.e., 12 months, 6 months, or 1 month). After the initial term, the subscription will continue on a month-to-month basis unless canceled.
- Three-Month Minimum Commitment: The subscription requires a minimum commitment of three months. After the initial term, the subscription will continue on a month-to-month basis unless canceled.
- Medical Eligibility: If, after reviewing your lab results, our medical team determines that you are not eligible for the Female Hormone Optimization Program, your subscription will not be activated, and you will not be charged the monthly subscription fee.
